Sciatica vs. Piriformis Syndrome: What’s the Difference?

Pain that starts in your lower back or buttocks and travels down your leg may be related to sciatica or piriformis syndrome. Although these conditions can cause similar symptoms, they differ in where the irritation may originate. [1,2]

What Is Sciatica?

Sciatica is pain that travels along the sciatic nerve, often from the lower back down one leg. It may occur when nerve roots in the lower spine become irritated or compressed.

A herniated disc, spinal stenosis, and other spinal conditions can contribute to sciatica. Symptoms may include shooting pain, tingling, numbness, or weakness in the affected leg. [1]

What Is Piriformis Syndrome?

Piriformis syndrome occurs when the piriformis muscle in the buttocks may irritate or compress the nearby sciatic nerve. This muscle helps control hip movement.

Symptoms may include deep buttock pain, discomfort that travels down the leg, and pain that worsens with prolonged sitting or certain hip movements. [2]

What’s the Difference?

The main difference is the potential source of nerve irritation.

  • Sciatica: May originate from irritation or compression involving the nerves in the lower spine.
  • Piriformis syndrome: May involve irritation of the sciatic nerve near the piriformis muscle in the buttocks.

Because symptoms can overlap, pain location alone may not be enough to distinguish between the two conditions. A healthcare provider may assess spinal and hip movement, muscle strength, sensation, and reflexes to help determine the potential cause. [1,2]

When Should You Seek Treatment?

If leg pain, tingling, or numbness persists or interferes with daily activities, an evaluation may help identify potential causes and guide treatment. A chiropractor or physical therapist may assess your symptoms, movement, and function to help determine appropriate care or whether referral to another healthcare provider is needed. Seek immediate medical attention for new bladder or bowel control problems, numbness around the groin, or significant or worsening leg weakness, as these may indicate serious nerve compression. [1]

When Should You Seek Treatment?

If leg pain, tingling, or numbness persists or begins interfering with daily activities, an evaluation may help identify potential causes and guide treatment. Again, seek immediate medical attention if you experience new bladder or bowel control problems, numbness around the groin, or significant or worsening leg weakness, as these may indicate serious nerve compression. [1]

The Bottom Line

Sciatica and piriformis syndrome can both cause pain that travels down the leg, but the potential source of irritation differs. Because other conditions can produce similar symptoms, a professional evaluation may help identify contributing factors and guide treatment.
